As such, when a concern arises pertaining to a person’s services, it is best to attempt to resolve the matter by speaking with supervisory staff from the applicable service/program. When that is unsuccessful, or not possible for some reason, please feel free to submit a Service Concern form by completing the fields below. In the “I’m Interested in” field, select Service Satisfaction Comment/Concern and describe your concern in the message box. Arc GLOW Quality Improvement staff will then provide support in following up and addressing the matter. Thank you!
